With verbal consent, health information may be given to designated persons involved in your care.

SmithLife Homecare may use or disclose individually identifiable health information without consent, authorization or opportunity to agree or disagree for other reasons, including those uses and disclosures required or permitted by law such as: legally mandated public health activities; circumstances involving abuse or neglect; health oversight activities; judicial and administrative proceedings; law enforcement purposes; coroners and medical examiners; organ donation; and other purposes permitted by law.

Individual Rights

Except in limited circumstances, an individual may review or obtain a copy of protected health information. If you request copies of your protected health information, SmithLife Homecare may charge $ .50 per page. You have the right to request additional restrictions on the use and disclosure of health information, to amend that health information and to request that SmithLife Homecare communicate by a different means about health information; however, SmithLife Homecare is not legally required to agree with such a request. About our Online Advertising Practices
